Verifies patient demographics. Focuses on items impacting payments, star ratings, and value-based purchasing. Reviews accuracy of M items and GG items scoring. Reviews the accuracy of physical assessment to support homebound status and medical necessity.
PLAN OF CARE REVIEW
Ensure goals and interventions address patient needs provided by focus of care and comprehensive assessment. Compliance of conditions of participation guidelines. Verify eligibility for homebound status.
